The administration and staff at University Hospital and Medical Center take pride in the many conveniences we offer to our patients and visitors. A Complimentary Valet parking service is available upon arrival to the hospital, Monday through Friday from 8 a.m. to 6 p.m. The valet service is located at our inpatient/outpatient registration department on the north side of the main hospital.

Upon admission to University Hospital and Medical Center, we will ask you for some information and ask that you sign your consent to receive treatment.

Even though you sign consent for treatment forms when admitted, others may be necessary for specific procedures during the course of your hospitalization. It is important that you understand these consents. Any questions should be directed to your physician.

Requirements for all Registrations

Upon arrival in the Registration department, you will be asked to verify all demographic and insurance information. You will be required to provide the following information:

  • Health insurance cards and information including address, telephone number, and policy number
  • Photographic identification card
  • Social Security card
  • A valid prescription or physician order for all tests
  • Referral or authorization numbers, if required by your insurance
  • A note giving "permission to treat" from a parent or legal guardian if the minor will be unaccompanied by the responsible party.
  • Advance Directives

Under Florida Law, every adult has the right to make certain decisions concerning their medical treatment. Advance Directives allows for your rights and personal wishes to be respected even if you are too sick to make decisions yourself. You have the right to decide whether to accept or reject medical treatment, including whether or not to continue medical treatment and other procedures that would prolong your life artificially.

You may spell out these rights in a "Living Will" containing your personal directions about life-prolonging treatment in the case of serious illness that could cause death. You may also designate another person as a healthcare surrogate who may make decisions for you if you become mentally or physically unable to do so. This surrogate may function on your behalf for a brief time or longer, for a life threatening or non-life threatening illness. Any limits to the power of the surrogate in making decisions for you should be clearly expressed. If you so request, the hospital will provide you with Advance Directive forms - the documents for designating a healthcare surrogate and specifying a "Living Will." If you have already made these arrangements, please provide us with a copy as soon as possible, so that it may be placed in your medical record.

Personal Belongings

We strongly advise you to leave - or send - home your valuables, such as keys, credit cards, jewelry and money. You may decide to deposit valuable items in a "valuables envelope," which can be stored in the hospital safe. The hospital is not responsible for money and other valuables kept in your room. Ask your nurse to contact Security to retrieve valuables that have been stored in the hospital safe. Please keep your dentures, eyeglasses and hearing aids in protective containers when not in use. The hospital is not responsible for their loss or damage. Please be particularly careful not to leave these items on your food tray.

The Health Insurance Portability and Accountability Act (HIPAA) is a federal law passed by Congress to address many healthcare issues including controlled access to protected health information. While patient privacy has always been an organizational priority, the administration and staff at University Hospital and Medical Center have taken great measure to ensure compliance with the new guidelines.
